Insurance

Pank Chiropractic accepts many types of insurance.  As a courtesy to our patients we will call to verify benefits and eligibility at the first visit.  Ultimately it is the patients responsibility to know what their insurance benefits are.

Payment At the Time of Service

All copayments and deductible amounts are due on the same date of service.  There will be no more than one date of service kept outstanding on an account.  This amount will have to be paid before the next date of service before the patient will be seen.  Cash, check, and credit card are accepted.

Fees

The same date of service fee for:

• New Patient Exam and Adjustment- $95.00
• Spinal Adjustment $45.00
• X-rays: Spinal $85.00 Extremity $100.00
• Electrical Muscle Stimulation $20.00
• Ultrasound $25.00
• Kineseotaping $8.00
• Spinal Decompression $35.00 per session.
• Spinal Adjustment with Decompression $60.00
